PERSONAL INFORMATION COLLECTION NOTICE – “CLUBPASS SIGN IN TO WIN COMPETITION PROMOTION”
(required under Australian Privacy Principle ("APP") 5)
ClubPASS Holdings Pty Ltd (ACN 604 486 115) as the trustee for ClubPASS Trust (ABN 28 507 545 832) (ClubsNSW, we, us, our) will collect, store and use the personal information you have provided to us pursuant to the terms of this Collection Notice and our Privacy Policy.

The primary purpose for which we are collecting your personal information in this webform is to facilitate your participation in, and our management of, the “Second Chance Draw” random draw to win $5,000 in connection with the “ClubPASS Sign In To Win Competition Promotion” (the Promotion) and to enable us to notify you if you win the Second Chance Draw.
You agree that we may also use your personal information for related secondary purposes including:
- to provide you with information (including via email and SMS) about the Promotion;
- as required or permitted by any law (including the Privacy Act); and
- for any other purpose stated in our Privacy Policy.
ClubsNSW will collect the following personal information from you:
- first name;
- last name;
- date of birth;
- email address; and
- phone number.
Providing us with the requested personal information is not required by law. However, if you do not provide us with the personal information we ask for, or the information provided is incorrect or incomplete, you will not be able to participate in the Second Chance Draw.
We may disclose your personal information to the following organisations for the purposes described above (as well as otherwise permitted or required under Australian law):
- randomdraws.com.au by Trade Promotions and Lotteries Pty Ltd ACN 601 297 330 (https://www.randomdraws.com/au/);
- the Eligible Club at which you participate in the Promotion;
- our related entities;
- our employees, contractors and external service providers (and those of our related entities);
- as required or permitted by any law (including the Privacy Act); and
- any other organisation as covered in our Privacy Policy.
It is unlikely that we will disclose your personal information to any recipients that are based overseas, however to the extent that we do (for example third party cloud storage providers and other service providers), we have taken reasonable steps to ensure that any overseas recipient of your personal information complies with the APPs. By accepting this Collection Notice, you consent to us disclosing your personal information to overseas recipients.
